4/16/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Anthony. Upon arrival to the home setting, Anthony appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C engaged Anthony in a discussion to determine behaviors since the last session. Ms. Cosby discussed with QMHP-C on how he continues to be disrespectful towards her and his siblings. 2. Behavior: Anthony participated in the session and needing several prompting from QMHP-C due to him becoming aggressive when discussing information presented by his mother. Anthony indicated to QMHP-C he utilizes the skills that are being taught to him by his mother; however, at times, his mother just takes him to another level and he just ignores her and that is something that she does not like. 1.Intervention: QMHP-C began the session by inquiring about Anthony’s current thoughts, feelings and emotions. QMHP-C engaged Anthony in a discussion with Anthony on how he needs to show leadership and become more responsible as it relates to his mother. QMHP-C reviewed the recent incident that occurred in the home. QMHP-C allowed Anthony the opportunity to implement self-calming strategies prior to the session’s beginning and encourage positive coping skills. QMHP-C immediately confronted Anthony’s aggressive behaviors of him becoming defensive when his mother is redirecting him to complete a task or come home when he is supposed to. QMHP-C processed with Anthony on how he needs to be a role model for others in the and in the community and show respect to his mother for she is doing all she can to ensure that he has a nice environment and living arrangement. 2.Intervention: QMHP-C encouraged Ms. Cosby to confront him behaviors in a stern manner. QMHP-C modeled the appropriate way for Ms. Cosby to confront Anthony’s behavior. QMHP-C engaged Ms. Cosby and Anthony in a discussion to allow them both to identify triggers that resulted in the situation. QMHP-C assisted Anthony and Ms. Cosby with identifying areas that could have been avoided to prevent his behavioral situations. QMHP-C provided them with alternative ways to manage conflicts. QMHP-C shared with Anthony possible consequences for his behavior exhibited towards his mother. QMHP-C asked Anthony to demonstrate the appropriate way he should have responded to Ms. Cosby instead of becoming aggressive. 1.Response: Anthony was receptive to QMHP-Cs’ interventions and services provided during the session. Ms. Cosby appeared to be visibly upset and frustrated with Anthony’s behavior. Anthony had limited communication with his mother. He communicated with QMHP-C but showed little remorse for his actions and aggressive behaviors towards his mother. He continues to blame shift his reactions and minimize his behaviors. Ms. Cosby struggles to confront Anthony appropriately for she will automatically call the police on the him. Ms. Cosby becomes provoking as she confronts Anthony’s behaviors due to the frustration that she has regarding Anthony’s constant negative/aggressive behaviors. Anthony verbalized that his mother gets on his nerves sometimes and when he says something to his mother about her over exaggerating in some of the things that she be reporting to QMHP-C is not the truth. 2. Response: Anthony stated to QMHP-C his mother just goes to the extreme sometimes and he does not even be doing anything. He furthered discussed with QMHP-C his mother believes his siblings all the time over him when he really does not even be doing anything to them while providing QMHP-C with examples of how his mother treats him differently from his siblings. Ms. Cosby stated to QMHP-C that is not the truth it is the fact that Anthony thinks that he can do what he wants to and think that it is okay. Anthony stated to QMHP-C, “She just in the way and he does way too much”.
